According to Gianluca Di Marzio, the 22-year-old winger will initially sign on a two-year loan.
Juventus will pay €2m for the first year of his loan, then €8m for the second year.
At the end of the two years, Juve will have an obligation to buy Chiesa for €40m with a few conditions.
The first condition will be that Juventus make the Serie A top four.
Secondly, Chiesa will have to appear in 60% of matches (playing 30 minutes or more) for Juventus OR achieve 10 goals and 10 assists.
In total, the operation could be worth €50m including bonuses.
The one sticking point that threatened the deal on Sunday night was Chiesa’s contract renewal with Fiorentina.
Chiesa wanted his salary adjusted to be in line with what he will earn at Juventus. At first, Fiorentina refused, but a compromise has now been reached.

The talented Italy international has already made over 150 appearances for Fiorentina’s first team.
Juventus needed some outgoings before they could commit to a deal for Chiesa and made progress on that this weekend.
Meanwhile, Fiorentina could reportedly bring in Napoli legend Jose Callejon – still a free agent – to replace Chiesa.
