Candreva, 33, will sign a four-year deal with Sampdoria after the two clubs agreed terms on Thursday.
The Italian international joined Inter from Lazio in 2016 for a reported fee of €22m plus bonuses.
He remained an active part of Antonio Conte’s side last season, making 40 appearances in all competitions.
Candreva has played 54 matches for Italy and represented his country at the 2014 World Cup in Brazil.
Candreva’s move to Sampdoria has now been made official.
