Milan will pay €10m for the loan then €25m for the permanent deal and bonuses.
In total, the operation for Tonali is worth up to €30m, an important investment by the Rossoneri.
Tonali pressed for the move in recent days after Inter boss Antonio Conte cooled his interest.
In the next few hours, Milan and Brescia will exchange the relevant paperwork to finalise the deal.
Tonali will sign a five-year contract with Milan.
Three-time Italy international Tonali leaves Brescia after rising through their youth ranks and making 89 first-team appearances.
